Application for In-Car Driver Education and Parent/Guardian Agreement

Please read ALL the information below before enrolling your son/daughter in the in-car phase of the driver education program:

Before the in-car driver education phase, the following items should be completed:

1. Students be taking (or have completed) the CLASSROOM Driver Education program.
2. Parents certify that their child has completed at least two hours of guided practice prior to starting in-car instruction.
3. The student has obtained a Lerner's Permit.

Scheduling In-Car at Bishop O'Connell:

Parents, please complete the form below to schedule in-car driver education at Bishop O'Connell. Students are scheduled on a combination first come-first serve and by their birthday throughout the school year. There is a $350 charge for this program.

Licensing Procedures:

Students with a learner’s permit are eligible for a 180-day provisional license if they:

1. Are at least 16 years and 3 months old
2. Have held a learner’s permit for 9 months
3. Have successfully completed a state-approved driver education program which includes classroom and in-car phases
4. Have parent/guardian permission. Parents must certify their child:

• Is properly insured on validation date of provisional license
• Has no court sanctions
• Has driven for at least 45 hours, with at least 15 of those after sunset (Submit log from “45-Hour Parent/Teen Driving Guide”)
• Is in good academic standing at school

(Permission form will be provided by the in-car instructor.)

After receipt of the parent permission form and 45-hour log, the in-car instructor will issue the student a 180-day provisional driver’s license. This document, accompanied by a valid learner’s permit, allows the student to operate a motor vehicle with no adult supervision. If at the time of course completion you feel your child is not ready receive the provisional license, please notify the in-car instructor. The instructor will hold the provisional license until we receive your permission to validate it.

The school will send a copy of the provisional license to the DMV. The Juvenile court will then notify you by mail, within the 180-day period, as to when you and your child should appear before the judge to receive the permanent license.

It should be emphasized that successful completion of a driver education program does not imply that your child has been “crashed proofed” or is a safe, accomplished driver. Many skills are practiced during the course; however, we provide at least 7 periods of actual driving instruction. Please recognize that your child needs a lot more guided experience to become an effective and responsible driver.

Parents will be asked to verify that your child has completed two hours of guided driving practice prior to starting in-car lessons.

Payment Authorization
By completing and submitting this form, I authorize the school to charge any applicable fees associated with this request through FACTS Incidental Billing. I understand that FACTS will notify me of the charge and that the charge will be processed to my payment method on file within approximately ten (10) days unless I update my payment information beforehand. I agree to be responsible for all fees incurred and understand that payment will be collected through FACTS Incidental Billing in accordance with the school’s billing policies. By checking this box I agree that refunds will be considered on a case by case basis.
