Required Sports Forms


The required medical forms for registered freshmen were mailed Friday, May 18.  Please consult the overview of required forms if you have questions.  All forms should be submitted to the Office of Admissions.

Returning Students
All returning students must submit completed forms to Mr. Tillson (Athletic Training Office) before the first day of practice.

All Students
An original back-to-back parent consent form is now required. Forms are available in the athletic office, athletic training room, and normally handed out at team sign-up meetings.  If you are unable to get an original form, you may print the form from the links here, but please print the sides back-to-back. Sides printed on separate paper will not be accepted. Consent forms submitted by fax or as an e-mail attachment are not acceptable.

Effective 7/1/2011, Virginia law and the Arlington Diocese require that in order to participate in any athletic activity, each student-athlete and their parent/guardian shall review information on concussions provided by the school. After reviewing these materials describing the effects of concussions, each student-athlete and their parent/guardian shall sign a statement, which is on the parent consent form, acknowledging review and understanding of such information. This information is also available as a handout from the athletic trainer.

An original of the O’Connell medical history/physical form (Form D) is preferred but not required. Other forms your doctor prefers to use are acceptable. It is recommended that you make a copy to keep for your records. Physical forms received by fax are acceptable if necessary to get the forms in before the first practice. The athletic department fax number is 703-237-1401. Physical forms submitted as an e-mail attachment are not acceptable.
