Required Sports Forms
All forms should be submitted to the Office of Admissions.
All forms should be submitted to Mr. Tillson (Athletic Training Office) at least a week before the first day of practice. Bringing forms to the first practice will delay a student’s clearance to participate.
Parental Consent Form - Please submit an original back-to-back parent consent form. Forms are available in the athletic office, athletic training room, and normally handed out at team sign-up meetings. If you are unable to get an original form, you may print the form from the links below, but please print the sides back-to-back. Consent forms should be submitted by mail or brought in.
Concussion Information - Virginia law requires that in order to participate in any athletic activity, each student-athlete and their parent/guardian shall review information on concussions provided by the school. After reviewing these materials describing the effects of concussions, each student-athlete and their parent/guardian shall sign a statement, which is on the parent consent form, acknowledging review and understanding of such information. This information is available as a handout from the athletic trainer and from the links below. There is a separate handout for athletes and parents. There will also be a more informative presentation offered at the beginning of the year for all athletes and parents.
Medical History/Physical Form - An original of the O’Connell medical history/physical form is preferred but not required. Other forms your doctor prefers to use are acceptable. It is recommended that you make a copy to keep for your records. Forms should be submitted by mail or brought in.
Physical forms received by fax are acceptable if necessary. The athletic department fax number is 703-237-1401.
