Required Sports Forms
Bishop O'Connell is now using a paperless electronic health records system from Magnus Health, which you can find through your MyDJO portal on the school web site. Health forms will no longer be submitted directly to the admissions office, the nurse, or the athletic trainer. Once you are on Magnus Health you can click Support, then Parents for links to Getting Started video, and answers to some frequently asked questions.
Parents of students trying out for any sport must complete the "Sports Only Requirements" in addition to the Vital Health Record, and other forms required by the school nurse.
The "Sports Only Requirements" include:
1. Pre-participation Physical Exam
You can download and print the form to take to your doctor. DOWNLOAD Physical Exam Form HERE. Any other forms the doctor chooses to use will still be acceptable.
The completed physical form can be uploaded by scanning and uploading the file, uploading a picture of the form, or faxing/mailing the form to Magnus with the student’s specific cover letter.
If you have previously submitted a paper physical form, it has been uploaded to the student’s Magnus file.
2. Parental Consent for Athletic Participation and Acknowledgement of Risks
This is an electronically signed statement.
3. Heads Up Concussion Fact Sheet for Parents
Answer “yes” after a parent has read the flyer.
4. Heads Up Concussion Fact Sheet for Athletes
Answer “yes” after the student has read the flyer.
5. Student Driving to Athletic Events
This is an electronically signed statement. Complete if you give permission for your child to ride with someone else to a team activity.
These requirements are for athletes only, so only the parents of students who have signed up for a team will be able to see and complete these requirements.
PLEASE NOTE: Students who have not played on a team before must give their name to a coach before tryouts so it can be entered on Magnus as an athlete.
Remember, it takes time to check and approve everyone’s forms, so waiting until the first day of practice will delay your child being cleared to tryout. Thank you for your patience and cooperation during this transition.
